Why Have You Been Referred?

If you have been absent from work due to ill-health or have an underlying medical condition, or disability that may prevent you from carrying out your normal full range of duties effectively, your company will ask us for advice on how best to support you in your return to work, or to enable you to undertake your job effectively.

Your manager or a member of your Human Resources (HR) Department will have discussed the specific purpose of the referral with you prior to the appointment.

What About My GP?

Any communication with your GP will be with your consent, additional to the consent you have
given to be referred to Occupational Health. If a medical report on your condition is required from your GP (or hospital consultant), this will only be sought with your written consent, under the Access to Medical Reports Act, 1988.

All information remains confidential and bound by UK Data Protection Regulations. It will not be
given to anyone without your informed consent, unless it is in the public interest, following a Court
Order or required by law e.g. a Notifiable Disease.

Attending and Rescheduling

If your appointment is in person it will take place at the Riccarton General Practice unless you have made arrangements otherwise. Please arrive 5-10 minutes before your appointment begins. Most appointments last 30 minutes.

If you wish to reschedule or cancel your appointment you should discuss this with your HR department or manager as soon as possible. In the case of cancellations they may be unable to make any changes in your interest and that of the company, without all the requisite information.
